ABSTRACT

Objective: To analyze the survival time of reported HIV/AIDS and influencing factors of Yunnan Province from 1989 to 2021. Methods: The data were extracted from the Chinese HIV/AIDS comprehensive response information management system. The retrospective cohort study was conducted. The life table method was applied to calculate the survival probability. Kaplan-Meier was used to draw survival curves in different situations. Furthermore, the Cox proportion hazard regression model was constructed to identify the factors related to survival time. Results: Of the 174 510 HIV/AIDS, the all-cause mortality density was 4.23 per 100 person-years, the median survival time was 20.00 (95%CI:19.52-20.48) years, and the cumulative survival rates in 1, 10, 20, and 30 years were 90.75%, 67.50%, 47.93% and 30.85%. Multivariate Cox proportional risk regression model results showed that the risk of death among 0-14 and 15-49 years old groups were 0.44 (95%CI: 0.34-0.56) times and 0.51 (95%CI:0.50-0.52) times of ≥50 years old groups. The risk for death among the first CD4+T lymphocytes counts (CD4) counts levels of 200-349 cells/μl, 350-500 cells/μl and ≥501 cells/μl groups were 0.52 (95%CI: 0.50-0.53) times, 0.41 (95%CI: 0.40-0.42) times and 0.35 (95%CI: 0.34-0.36) times of 0-199 cells/μl groups. The risk of death among the cases that have not received antiretroviral therapy (ART) was 11.56 (95%CI: 11.26-11.87) times. The risk for death among the cases losing to ART, stopping to ART, both losing and stopping ART was 1.66 (95%CI:1.61-1.72) times, 2.49 (95%CI:2.39-2.60) times, and 1.65 (95%CI:1.53-1.78) times of the cases on ART. Conclusions: The influencing factors for the survival time of HIV/AIDS cases were age at diagnosis in Yunnan province from 1989 to 2021. The first CD4 counts levels, antiretroviral therapy, and ART compliance. Early diagnosis, early antiretroviral therapy, and increasing ART compliance could extend the survival time of HIV/AIDS cases.

ABSTRACT

Objective: To explore the occupational hazards caused by three kinds of welding operations, and to provide data support for individual protection. Methods: In October 2020, the welding fumes, metal elements and welding arc generated by three welding operations of argon gas shielded welding (JS80 welding wire) , manual welding (ZS60A welding rod) and carbon dioxide shielded welding (907A flux cored wire) were collected and measured in the welding laboratory. The samples were analyze and compare in the laboratory, and the differences of the occupational hazard factors of the three welding operations were judged. Results: The concentration of welding fume produced by carbon dioxide shielded welding, manual welding (ZS60A electrode) , and argon gas shielded welding (JS80 welding wires) were 6.80 mg/m(3), 6.17 mg/m(3), and 3.13 mg/m(3), respectively. The effective irradiance of the welding arc outside the welding mask from high to low is manual welding (ZS60A electrode) , carbon dioxide shielded welding (907A flux-cored welding wire) , and argon shielded welding (JS80 welding wire) , respectively 1 010.7, 740.9, 589.5 μW/cm(2). The long-wave ultraviolet UVA intensity generated by argon shielded welding (JS80 welding wire) is the largest, which is 1 500 μW/cm(2). The content of Mn in the three welding operations is the highest, and JS80 welding wire has the highest Mn content of 128493.2 mg/kg. 907A flux cored wire has the highest Ti content, which is 24355.5mg/kg. The electrode ZS60A has the highest Cu content, which is 24422.12 mg/kg. Conclusion: The intensity of occupational hazards is different in the three kinds of welding operations, so the methods of personal protective equipment, field exposure assessment and health monitoring should be more targeted.

ABSTRACT

@#【Objective】To evaluate the feasibility of magnetization transfer(MT)magnetic resonance(MR)imaging for predicting the risk of intestinal fistula in patients with Crohn disease (CD). 【Methods】 The study prospectively enrolled 12 consecutive patients with CD and abdominal MT imaging were performed before elective surgery. The bowel wall MT ratio normalized to skeletal muscle was calculated;region- by- region correlations with the surgical specimen were performed. Histopathologic evaluation of fibrosis was executed by using Masson trichrome. Wilcoxon rank test , Spearman rank correlation, and receiver operating characteristic curve (ROC) were used for statistical analysis.【Results】Among 15 surgical intestinal segments from 12 patients,5 lesions were found with intestinal fistula and of them 12 bowel specimens were obtained. The other 10 intestinal segments were without complications and 23 bowel specimens were enrolled. The intestinal fistula bowel showed a significant higher (P=0.045) normalized MT ratio. ROC analysis revealed an area under the curve of 0.674(95%CI:0.537-0.811)for differentiating intestinal fistula bowel from the non- fistula one. The sensitivity, specificity was 93.3% and 51.1% with a cut-off value of 76.8%,respectively. For the Masson score, significant difference(P=0.012)was found between the complicated intestinal specimens and the non-complicated ones. Additionally, the normalized MT ration was statistical correlated with Masson score (r=0.708,P<0.001).【Conclusion】MT imaging could be a potential method to predict the risk of intestinal fistula in patients with CD.

ABSTRACT

OBJECTIVETo analyze the association between CT features and survival rate of GIST, and to elucidate the significance of CT features for prognosis.

METHODSClinical data of 38 patients with pathologically and immunohistochemically proven GISTs, including 11 patients at high biological risk, 13 at moderate risk, 10 at low risk and 4 at very low risk. Patients who underwent CT examination for primary tumors were included. Association between CT features and survival rate was examined.

RESULTSThe mean follow-up period of 38 cases was 42.6 months and the 3-year survival rate was 86.8%. Univariate analysis revealed that tumor growth pattern, diameter, lobulated shape, irregular margin, necrosis, ulceration, adjacent invasion, and liver metastasis were associated factors of 3-year survival rate. Circumference invasion and hepatic metastases predicted poor 3-year survival rate (P<0.05). Calcification and intensity were not associated with prognosis (P>0.05).

CONCLUSIONSCT can demonstrate the tumor growth pattern, size, shape, boundary, density, necrosis, hemorrhage, calcification, ulcer, enhance features and metastasis. CT can play an important role in estimating the survival rate of GIST.

ABSTRACT

OBJECTIVETo evaluate the efficacy and clinical significance of 64-multislice spiral computed tomography angiography(MSCTA) with image fusion for the anatomy of perigastric arteries.

METHODSA total of 53 patients underwent abdominal 64-MSCTA, among whom 26 patients with gastric cancer underwent gastrectomy. Using volume rendering techniques, computed tomography angiography(CTA) of perigastric arteries and the stomach were reconstructed respectively, and then the images were fused together. The branching pattern of the celiac trunk and the origins and courses along the stomach of the 10 perigastric arteries were assessed. The accuracy, sensitivity, and specificity of 64-MSCTA were determined based on intraoperative findings.

RESULTSCTA clearly showed the celiac trunk. The most common branching pattern of the celiac trunk was Michels type I( in 46 patients(86.8%). The anatomy of perigastric arteries and stomach could be clearly demonstrated from any angle according to image fusion. The left gastric artery and the right gastroepiploic artery were shown in 100%, the left gastroepiploic artery 94.3%(50/53), the right gastric artery 83.0%(44/53), short gastric artery 58.5%(31/53), posterior gastric artery 49.1%(26/53), the replaced left hepatic artery 15.1%(8/53). The accessory left hepatic artery, accessory left gastric artery and replaced right hepatic artery were all identified in 7.5%(4/53) patients. The accuracy of preoperative CTA in term of correctly identifying perigastric arteries ranged from 84.6% to 100%, the sensitivity 82.6% to 100%, and the specificity was 100% for all the perigastric arteries.

CONCLUSIONS64-MSCTA can clearly reveal individual perigastric arteries. The anatomy of the stomach and perigastric arteries can be shown in vivo by fused image, and can provide guidance for gastrectomy.

ABSTRACT

Objective To establish models to predict individual risk of essential hypertension and to evaluate and explore new forecasting methods. Methods To select data of 3054 community residents from a epidemiological survey and divided them into 4 : 1 (2438 cases and 616 cases) ratio in accordance with the balance of age and sex to filter variables, and to establish, test and evaluate the prediction models. Using artificial neural network (ANN) and logistic regression analysis to establish models while applying ROC to evaluate the prediction models. Results Forecast results of the models applying to the test set proved that ANN had lower specificity but better veracity and sensitivity than logistic regression. In particular, the Youden's index of the ANN2 came up to 0. 8399 which was distinctly higher than the other two models. When the area was under the ROC curve of logistic regression, the ANN1 and ANN2 models equaled to 0. 732±0. 026,0. 900±0. 014 and 0. 918±0. 013 respectively, which proved that the ANN model was better in the prediction about individual health risk of essential hypertension. Conclusion Our results showed that ANN method seemed better than logistic regression in terms of predicting the individual risk from hypertension thus supplied a new method to solve the forecast of individual risk.

ABSTRACT

OBJECTIVETo evaluate the effects of two sample preparation methods on two-dimensional polyacrylamide gel electrophoresis (2-DE)-based serum protein separation, and produce high-resolution and reproducible 2-DE images for identifying disease-related serum protein.

METHODSDirect solubilization and hot SDS methods were used separately to extract and handle the total proteins of serum samples from patients with colorectal carcinoma. Immobilized pH gradient 2-DE was used to separate the total proteins. After image analysis of silver-stained 2-D gels, 3 differential protein spots were identified by matrix-assisted laser desorption/time-of-flight mass spectrometry.

RESULTSThe total proteins treated with hot SDS method were used to perform 2-DE. 2-DE patterns with high resolution and reproducibility were obtained for human serum samples. 2-DE was performed 3 times for the samples treated by direct solubilization and hot SDS methods, respectively, resulting in the average number of spots of 675-/+46 and 702-/+49, respectively. The average matching protein spots were 573-/+42 and 623-/+52, with average matching rate of 85.3% and 89.6%, respectively. The average position deviation of matched spots in different gels was 0.85-/+0.30 mm and 0.81-/+0.28 mm in IEF direction, and 1.02-/+0.18 mm and 0.97-/+0.12 mm in SDS-PAGE direction. Mass spectrometry of the 2-D gels treated with hot SDS method generated high-quality mass spectra, and the sample preparation method allowed detection of relatively low abundance protein.

CONCLUSIONHot SDS method is more effective for human serum protein sample preparation and well-resolved, reproducible 2-DE profiles of human serum have been established in this study.
